NGN Case Studies: The Feature That Changes Everything

Welcome back! We’re at the halfway point of our NGN NCLEX-RN series, and today we’re tackling something that makes the Next Generation NCLEX genuinely different from anything that came before it — case studies.

If the new question types from Part 2 felt like a curveball, case studies are the full swing. But here’s the good news: once you understand how they work and you’ve practiced them, they become one of the most manageable parts of the exam.

What Is a Case Study on the NGN?

A case study on the NGN is exactly what it sounds like — a detailed patient scenario that you work through across multiple questions. Instead of answering one isolated question about a random patient, you’re following one patient through a clinical experience and making decisions at each stage.

This is much closer to real nursing than any multiple-choice question ever could be.

There are two types of case studies you’ll encounter:

📋 Standalone Case Studies

In a standalone case study, you’re given a patient scenario — history, assessment findings, vital signs, lab results, the works — and then you answer 6 questions based on that scenario.

Each question targets a different cognitive skill from the Clinical Judgment Measurement Model:

1. Recognize the relevant cues
2. Analyze what those cues mean
3. Prioritize the most urgent concerns
4. Generate appropriate solutions
5. Take the right action
6. Evaluate whether the intervention worked

You’re walking through the entire clinical thought process — start to finish — with one patient. It’s comprehensive, it’s connected, and it genuinely tests your nursing judgment in a way that isolated questions simply can’t.

📋 Unfolding Case Studies

These take things one step further — and they’re the ones that feel most like real nursing.

In an unfolding case study, the patient’s condition changes as you move through the questions. Maybe they come in with chest pain, and then new labs come back. Maybe their vitals shift. Maybe they respond to a medication — or they don’t.

With each new piece of information, you have to reassess, reprioritize, and adapt. You can’t answer question 4 based only on what you knew in question 1. The clinical picture keeps evolving, and so must your thinking.

Sound familiar? It should. Because that’s exactly what happens every single shift at the bedside.

Why Do Case Studies Trip People Up?

Here’s what happens to a lot of candidates: they’ve spent most of their study time on traditional multiple-choice questions. They’re comfortable with the format. They can move through questions quickly.

Then they hit a case study.

Suddenly there’s a long scenario to read. There are six connected questions. The patient’s condition is changing. There’s more information than they’re used to processing at once.

And they panic.

Here’s the thing — the panic is the problem, not the case study. The clinical content isn’t harder than what you’ve been studying. The difference is the format and the volume of information. Both of those things become completely manageable with practice.

How to Approach Case Studies Like a Pro

🔑 Read the scenario carefully — all of it.

Yes, it’s long. Yes, it takes time. Read it anyway. Every detail in that scenario is there for a reason. The vital signs matter. The medication history matters. That one lab value buried in the middle of the page? It might be the most important thing in the whole case.

🔑 Take notes as you read.

Use the on-screen notepad. Jot down key findings, abnormal values, anything that stands out. Organizing information as you go makes answering the questions significantly easier.

🔑 Remember which cognitive skill each question is targeting.

As you move through the six questions of a case study, keep the CJMM in mind. Ask yourself: “Is this asking me to recognize something? Prioritize? Evaluate?” Knowing what the question is looking for helps you focus your answer.

🔑 Don’t carry wrong assumptions forward.

This is a big one for unfolding case studies. Each question gives you updated information. Don’t hold on to your original interpretation if new data changes the picture. Stay flexible. Reassess. That’s what real nurses do.

🔑 Practice, practice, practice.

There is no substitute. Do as many full case studies as you can before your exam. Get comfortable with the length. Get comfortable with the evolving information. The more you practice, the more natural it feels.

The Numbers Behind the Exam

While we’re here, let’s talk about the overall structure of the NGN so you know exactly what you’re walking into:

Five hours is a long exam. Pacing matters. Don’t rush through case studies — but don’t let yourself get so bogged down that you’re scrambling at the end either. Practice under timed conditions so that managing your time feels automatic by exam day.
