The Next Generation NCLEX-RN Exam: A Complete Guide / Part 1 of 5
So… The NCLEX Changed. Here’s What You Need to Know.
If you’ve been sitting at your desk late at night, scrolling through study materials, and thinking “wait… this doesn’t look like what I expected” — you’re not imagining things. The NCLEX-RN has changed, and it’s changed in a pretty significant way.
The Next Generation NCLEX (NGN) launched in April 2023, and since then, nursing graduates everywhere have been trying to wrap their heads around what it actually means for their exam prep. Some people are stressed. Some are confused. And honestly? That’s completely understandable.
But here’s the thing — you can absolutely pass this exam. And by the time you finish reading this series, you’re going to feel a whole lot better about where you stand.
Let’s start from the beginning.
Why Did the NCLEX Change in the First Place?
The National Council of State Boards of Nursing (NCSBN) didn’t change the exam to make your life harder. They changed it because nursing itself has changed. Patients are sicker. Clinical environments are more complex. And the reality is that being a safe nurse today requires a lot more than just knowing the right answer to a multiple-choice question.
It requires thinking on your feet.
It requires looking at a patient, picking up on subtle changes, figuring out what’s most urgent, and acting — all while keeping that patient safe.
That’s what the NGN is designed to measure. And when you think about it that way, it actually makes a lot of sense.
The NGN isn’t trying to trick you. It’s trying to make sure you’re ready for the real thing — the bedside, the floor, the patients who are counting on you.
The Heart of the NGN: Clinical Judgment
If there are two words you need to burn into your brain right now, they are clinical judgment.
Everything about the NGN revolves around a framework called the NCSBN Clinical Judgment Measurement Model (CJMM). It sounds complicated, but it’s really just a structured way of describing how nurses think through patient situations.
Here’s how it breaks down:
|
# |
Cognitive Skill |
In Plain English |
|
1 |
Recognize Cues |
What information actually matters here? |
|
2 |
Analyze Cues |
What does this information tell me about my patient? |
|
3 |
Prioritize Hypotheses |
What’s most likely going on — and what’s most dangerous? |
|
4 |
Generate Solutions |
What can I do about it? |
|
5 |
Take Action |
What should I do right now? |
|
6 |
Evaluate Outcomes |
Did what I did actually help? |
Think about how naturally this mirrors what you do — or will do — at the bedside. You walk into a patient’s room, notice something, think about what it means, figure out what to do first, act, and then check whether it worked.
The NGN is essentially asking you to demonstrate that you can do exactly that.
How Is the NGN Different From the Old NCLEX?
Let’s put it side by side so you can see exactly what changed:

The shift is real — but notice that the foundation hasn’t changed. You still need to know your pharmacology, your disease processes, your lab values, your priorities. The NGN asks you to apply that knowledge in a deeper, more meaningful way.
Coming Up Next…
In Part 2, we’re diving into the brand-new question types on the NGN — including the ones that catch most candidates off guard. You’ll want to know what these look like before you sit down at that exam screen.
Stay tuned — and keep studying! 📚
